When reporting an issue, include these 6 essential details

1)  User ID & Name – Example: bxxo_karthi_xxx, Karthi (Batch 20)  
2)  Hostname & Port – Example: localhost:70xx
3)  Issue Description – Clearly explain the problem (error messages, what’s not working).
